In this role, you will provide high-quality, resident-centered care while collaborating with physicians, nursing staff, and interdisciplinary team members to ensure the best possible outcomes for our residents. Key Responsibilities Provide and oversee direct nursing care in accordance with physician orders and individualized care plans Administer medications, treatments, and specialized nursing procedures safely and accurately Assess residents and monitor for changes in condition Communicate resident status updates to physicians, families, and appropriate team members Maintain accurate and timely clinical documentation Supervise and support LPNs and CNAs Respond appropriately to resident needs, concerns, and emergencies Participate in care planning meetings and interdisciplinary team discussions Promote resident dignity, comfort, safety, and quality of life Ensure compliance with all federal, state, and facility regulations Utilize PointClickCare (PCC) and other clinical systems as needed Qualifications Current, unrestricted Registered Nurse (RN) license in South Carolina Current CPR certification Must maintain all required continuing education and licensure requirements Must remain in good standing with the South Carolina Board of Nursing Experience in skilled nursing, rehabilitation, or long-term care preferred Experience with PointClickCare (PCC) preferred Strong clinical assessment, leadership, communication, and organizational skills What Makes You a Great Fit Compassionate and resident-focused Strong clinical judgment and critical thinking skills Dependable and professional Excellent communication and teamwork abilities Passionate about delivering exceptional care to an underserved population Apply Today Join West Village Post Acute and become part of a team that's redefining post-acute care.
