In this role, you will provide high-quality nursing care, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary team to ensure the best possible outcomes for our residents. Key Responsibilities Provide direct nursing care in accordance with physician orders and individualized care plans Administer medications and treatments safely and accurately Monitor residents and promptly report changes in condition Maintain accurate and timely clinical documentation Supervise and support Certified Nursing Assistants (CNAs) Communicate effectively with residents, families, physicians, and interdisciplinary team members Respond appropriately to resident needs, concerns, and emergencies Promote resident dignity, comfort, and quality of life Ensure compliance with all federal, state, and facility regulations Utilize PointClickCare (PCC) and other clinical systems as needed Qualifications Current, unrestricted LPN license in South Carolina Current CPR certification Must maintain all required continuing education and licensure requirements Must remain in good standing with the South Carolina Board of Nursing Experience in skilled nursing or long-term care preferred Experience with PointClickCare (PCC) preferred Strong clinical, communication, and organizational skills What Makes You a Great Fit Compassionate and resident-focused Dependable and professional Strong communication and teamwork skills Positive attitude and strong work ethic Passionate about providing exceptional care Apply Today Join West Village Post Acute and become part of a team that's redefining post-acute care.
