In this role, you will provide direct resident care, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary team to ensure the highest quality outcomes for our residents. Key Responsibilities Provide direct nursing care in accordance with physician orders and individualized care plans Administer medications and treatments safely and accurately Monitor residents' conditions and report changes promptly to appropriate clinical staff Maintain accurate and timely clinical documentation Supervise and support Certified Nursing Assistants (CNAs) Communicate effectively with residents, families, physicians, and interdisciplinary team members Respond to resident needs, concerns, and emergencies appropriately Promote resident dignity, comfort, and quality of life Ensure compliance with all federal, state, and facility regulations Utilize PointClickCare (PCC) and other clinical systems as needed Qualifications Current, unrestricted LPN license in South Carolina Current CPR certification Must maintain all required continuing education and licensure requirements Must remain in good standing with the South Carolina Board of Nursing Experience in skilled nursing or long-term care preferred Experience with PointClickCare (PCC) preferred Strong clinical, communication, and organizational skills What Makes You a Great Fit Compassionate and resident-focused Dependable with a strong work ethic Excellent communication and teamwork skills Strong clinical judgment and attention to detail Passionate about providing exceptional care to an underserved population Apply Today Join Reedy River Post Acute and become part of a team that values your hard work, supports your professional growth, and recognizes the difference you make every day.
