In this role, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary team to ensure outstanding resident outcomes. Key Responsibilities Provide direct nursing care in accordance with physician orders and individualized care plans Administer medications and treatments safely and accurately Monitor resident conditions and promptly report changes Maintain accurate and timely clinical documentation Supervise and support Certified Nursing Assistants (CNAs) Communicate effectively with residents, families, physicians, and interdisciplinary team members Respond appropriately to resident needs and emergencies Promote resident dignity, comfort, and quality of life Ensure compliance with all federal, state, and facility regulations Utilize PointClickCare (PCC) and other clinical systems as needed Qualifications Current, unrestricted LPN license in South Carolina Current CPR certification Must maintain all required continuing education and licensure requirements Must remain in good standing with the South Carolina Board of Nursing Experience with PointClickCare (PCC) preferred Must be available to work 12-hour shifts Strong clinical, communication, and organizational skills What Makes You a Great Fit Compassionate and resident-focused Dependable and professional Strong communication and teamwork skills Positive attitude and strong work ethic Passionate about providing exceptional care to an underserved population Apply Today Join River Falls Post Acute and become part of a close-knit team where your contributions are appreciated, your growth is supported, and your work makes a meaningful difference every day.
