Job Description $10,000 Sign-On Bonus for eligible full-time and part-time new employees joining UW Medicine. UW Medical Center - Montlake's Social Work and Care Coordination and Palliative Care departments have an outstanding opportunity for an Inpatient Hospice Nurse Care Coordinator to join our team. DEPARTMENT DESCRIPTION With the goal of achieving coordinated care across the continuum this position specifically works as a catalyst to enhance processes for timely hospice & palliative care referrals, increase access to hospice care and maximize anticipated benefits of hospice for patients identified as hospice eligible in the acute care setting. WORK SCHEDULE Days Monday to Friday 8am-4:30pm PRIMARY JOB RESPONSIBILITIES Attend daily interdisciplinary rounding with palliative care and critical teams to identify patients in need of hospice education, access and care. In coordination with service line social worker, meets with patients/families to provide further information and resources about hospice. Educates hospital staff in relation to hospice services and regulatory requirements. Acts as inpatient liaison for community hospice agencies and assists with questions related to hospital to hospice admission and discharge processes as needed. Assists with maintenance of policies, procedures and admission criteria for hospice patients based upon CMS, community hospice partners and hospital policies. Tracks hospice enrollment for patients discharged to home and facility-based care. Identifies barriers and improves processes for patients referred to hospice in the community who did not enroll. Maintains and reviews patient records, charts and documents to support accuracy, quality measures, and best practice with regard to patients evaluated for hospice eligibility, hospice enrollment, and related hospital quality measures. REQUIREMENTS Master's degree in Nursing or related field. BSN with applicable experience may substitute for education. Three years of professional nursing experience. Case management and discharge planning; hospice case management or hospice nursing preferred. Effective interpersonal skills, leadership skills, and clinical expertise in the care of patients with a life-limiting illness. Ability to access information and document in electronic medical record. Current Washington State RN licensure at time of hire, and current BLS.
