Key Responsibilities Clinical Care & Supervision: Direct and monitor the day-to-day functions of nursing assistants Ensure quality care in accordance with resident care plans Admit, transfer, and discharge residents as needed Administer medications and treatments as prescribed Monitor and report changes in residents conditions Documentation & Reporting: Accurately complete and maintain residents medical records Record physician orders, chart treatments, and update care plans Prepare required forms, evaluations, and reports Leadership & Communication: Provide guidance and oversight to CNA staff Conduct rounds to ensure quality of care and staff compliance Communicate with residents, families, physicians, and interdisciplinary teams Participate in staff meetings, in-services, and training sessions Compliance & Safety: Ensure adherence to infection control, safety, and facility protocols Participate in audits, surveys, and quality assurance programs Maintain confidentiality of resident and facility information Qualifications Current and valid California LVN license CPR certification required Graduate of an accredited LVN program Knowledge of Medicare/Medi-Cal regulations preferred Experience with PointClickCare (PCC) preferred Ability to work independently and manage multiple priorities Strong written and verbal communication skills Must be able to lift up to 25 lbs and perform physical job duties as required Why Join Visalia Post Acute?
