Position Summary The Field Nurse Supervisor, Infusion is a leadership-level registered nurse responsible for overseeing home infusion field services and supervising nursing staff while also providing direct patient care. This role combines advanced infusion practice, field supervision, and patient-centered start-of-care responsibilities. The Supervisor partners with the Infusion Intake Team, Clinical Leadership, and Coordination staff to ensure seamless initiation of care, safe therapy administration, and compliance with all clinical and regulatory standards. This individual will serve as a resource and mentor for nurses, conduct supervisory visits, and ensure quality outcomes in the Lower Hudson Valley region. Responsibilities Conduct comprehensive patient assessments for infusion and home health services, including start-of-care and follow-up visits. Develop, implement, and oversee Plans of Care in collaboration with physicians, pharmacies, and multidisciplinary care teams. Provide hands-on infusion nursing including: Peripheral IV, PICC, midline, and central line care and access Implanted port (MediPort, PermaCath, etc.) access Infusion therapy preparation, administration, monitoring, and follow-up Educate patients and families on therapy administration, safety, and monitoring. Precept, evaluate, and supervise newly hired and current nursing staff in the field. Conduct supervisory visits, ensuring quality of care and adherence to standards. Provide oversight to home health aides and ensure compliance with care plans. Collaborate with the Infusion Manager, Intake Coordinators, and Clinical Directors to support staffing, scheduling, and case assignments. Maintain timely and accurate documentation in AlayaCare (EMR) or other paperless systems. Participate in paid on-call rotation to support after-hours clinical needs. Qualifications Active NYS Registered Nursing License & Registration Certificate (required) Minimum 3 years of RN experience in infusion therapy (hospital, clinic, or home infusion required) Supervisory or managerial experience preferred (precepting or charge RN also considered) Start-of-care home health experience strongly preferred Knowledge of Federal HIPAA regulations and standard nursing practices Strong assessment, prioritization, and time management skills Competence in clinical documentation, EMR (AlayaCare experience a plus) Reliable transportation and willingness to travel for patient visits Job Requirements BLS (American Heart Association) required; ACLS preferred Physical and QuantiFERON Gold/TB Clearance within 1 year MMR/Varicella titers or vaccination records Tdap and Hepatitis B vaccine series, titers, or declination Urine Drug Screen within the past year Flu vaccine or declination (seasonal) Malpractice insurance NPI number Two (2) professional references Compensation & Benefits Annual Salary: $120,000.00 Schedule: Mon-Fri 9 AM-5 PM with flexibility; Paid on-call rotation PTO & Benefits: 10 PTO days for the year (full package provided upon hire) Mileage reimbursement for field travel Opportunities for professional development and growth Equal Opportunity Statement Always Compassionate Health is committed to providing equal employment opportunities and maintaining a workplace free of discrimination and harassment.
