About Mass General Brigham Home Care Mass General Brigham Home Care provides post-acute, medically necessary skilled care in the home to help homebound patients recover from illness, surgery, or injury and manage exacerbations of chronic disease. Our clinicians are highly skilled professionals who function independently while collaborating as part of a strong interdisciplinary team to deliver the highest quality patient-centered care. What Makes Home Care Physical Therapy Different? As a Physical Therapist on our Paul Revere team, you will: Deliver 1:1 physical therapy care in patients homes functioning as a Case Manager Evaluate patients and develop personalized treatment plans Focus on mobility, strength, fall prevention, and functional independence Assess home environments to promote safety and independence Educate patients and caregivers on recovery strategies and long-term mobility Collaborate with nurses, therapists, physicians, and social workers to support recovery You wont just treat symptoms youll help patients regain independence and confidence in their own environment. Primary Coverage Area Most of your time will be spent in Jamaica Plain, while the Paul Revere team also supports the following communities: Dorchester Mission Hill Roxbury While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress-free as possible! Schedule & Workload Full-time (40 hours) MondayFriday schedule Average of 56 visits per day Dedicated time for documentation, chart review, visit preparation, and care coordination Set your own schedule by managing your own appointments, all while providing high-quality, one-on-one patient care. Job Summary Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs. Key Activities Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes Daily documentation is accurate and timely Application of standard treatment procedures Teaching is provided to patients and caregivers Qualifications Qualifications and Experience One year of experience in acute care, rehab or homecare as a Physical Therapist Registered with the Board of Registration in Massachusetts.
