You will play a key role in promoting healing, preventing complications, and improving overall resident outcomes. Key Responsibilities Provide direct wound care and skin treatments in accordance with physician orders Assess, monitor, and document wound progress and treatment effectiveness Assist in developing and modifying individualized care plans Collaborate with the interdisciplinary team to ensure optimal outcomes Educate staff on proper wound care techniques and prevention strategies Maintain accurate and compliant clinical documentation Ensure adherence to all regulatory, safety, and infection control standards Qualifications Active, unencumbered LPN license in South Carolina Graduate of an accredited Practical Nursing program Current CPR certification required Ability to work 12-hour shifts Experience with PointClickCare (PCC) preferred Prior wound care experience strongly preferred Strong clinical judgment and attention to detail Passion for delivering compassionate, resident-centered care Why Join Chandler Creek Post Acute?
