Your responsibilities include: Deliver direct nursing care in accordance with physician orders and individualized care plans Safely administer medications, treatments, and nursing procedures Supervise and support Certified Nursing Assistants (CNAs) Monitor resident conditions and promptly communicate changes to physicians and the interdisciplinary team Complete accurate, timely, and compliant clinical documentation Admit, transfer, and discharge residents as needed Participate in care planning, quality assurance, and interdisciplinary meetings Maintain infection prevention and safety standards Support resident rehabilitation and restorative nursing programs Foster a positive, respectful, and compassionate environment for residents and families Provide leadership, mentorship, and guidance to nursing assistants throughout your shift Qualifications We're looking for candidates who have: Current California LVN license in good standing Graduate of an accredited LVN program Current CPR Certification Skilled Nursing Facility or Post-Acute experience preferred Experience with PointClickCare (PCC) is a plus Excellent clinical judgment, communication, and organizational skills A compassionate, dependable, and team-oriented approach to resident care Physical Requirements This position requires standing, walking, lifting, pushing, pulling, and assisting residents throughout the workday.
