The LifeSharing Coordinator is responsible for ensuring, coordinating and facilitating quality support and services for individuals and their families within the program and their providers. Utilizing a Person-Centered approach, this position is responsible for developing, implementing, monitoring and modifying assessments specific to behavioral and clinical based Individual Support Plans. The Lifesharing Coordinator oversees the proper documentation of services and safety in accordance with applicable regulations. Job Details: Full time, non-exempt position $24.50 per hour Qualifications: One of the following: High school diploma or equivalent and six years experience working directly with persons with intellectual disabilities Associates degree with sixty (60) credited hours and four years experience working directly with persons with intellectual disabilities Bachelors degree and two years experience working directly with persons with intellectual disabilities Masters degree and one year experience working directly with persons with intellectual disabilities Ability to successfully complete and maintain the required Medication Administration, First Aid, CPR certifications and other trainings mandated by regulation within specified timeframes as applicable Effective communication and proficient computer skills Valid drivers license and daily access to a privately maintained and insured vehicle Successful completion of the pre-employment process, including clearances and verifications Keystone Human Services prohibits discrimination against qualified individuals based on their status as protected veterans or individuals with disabilities and prohibits discrimination against all individuals based on any category protected by applicable federal, state, or local law.
