Intake Coordinator Location: Rochester, NY (on site) Pay: $55,000.00 - $60,000.00 per year ***The position is based on site at our Rochester, NY office (Mon-Fri). Description: The Intake Coordinator serves as the point of contact through the intake and admissions process leading to start of care. This person will work closely with referral sources, account managers, personal care services team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process.
