Location New York, New York Shift: Evening (United States of America) Description: Don't Just Be an Advocate - Be an Ally for Patients At NYP, Patient Navigators spend their days navigating a multitude of adventures: on the phone, on the floor, dashing across the Emergency Department (ED) and diving into the paperwork. They might be one of the first few people a patient sees and possibly the last person to smile and wave. But every minute, they're always Putting Patients First. Now, you can make it possible. Patient Navigator - Ambulatory Care Network: Cornell - ED - Evenings Serve as a patient ally throughout the continuum of care. Coordinate the full breadth of the patient experience by ensuring that the patient is well educated and knowledgeable on navigating the healthcare system and has a comprehensive discharge plan once leaving the ED. Then, go a step beyond by providing non-clinical support to our patients by directing them to a primary, specialty doctor or the service(s) that they need. Tap into internal and/or community resources, like neighborhood clinics, to ensure that patients get the best clinical care and outcomes. The shift for this position will be Evenings, Tuesday - Saturday , working from 3:00 PM 11:00 PM. Preferred Criteria Experience working as a patient navigator, preferably in a hospital setting Bachelor's Degree from an accredited college or university Bilingual in English/Spanish Required Criteria High school diploma or GED Diverse and culturally sensitive customer services skills Knowledge of community patient and family needs and of the hospital and community resources to fill those needs Basic computer skills Join a healthcare system where employee engagement is at an all-time high.
