Social Services Assistant Full Time White Blossom Care Center San Jose, CA Starting Pay: $23/hour DOE Benefits Included White Blossom Care Center is seeking a compassionate and experienced Social Services Assistant to join our dedicated care team. If you have Skilled Nursing Facility (SNF) experience and a passion for supporting residents and families, we want to hear from you! Key Responsibilities Support the Social Services Director in planning and delivering resident-centered services Assist residents and families with care planning, transitions, and psychosocial needs Help complete required documentation, assessments, and progress notes in compliance with state and federal regulations Communicate with interdisciplinary team members to ensure resident needs are met Advocate for resident rights and support their emotional wellbeing Participate in discharge planning and community resource coordination Qualifications Skilled Nursing Facility experience required Knowledge of state & federal regulations related to longterm care social services Strong communication, documentation, and organizational skills Compassionate, resident-focused, and able to work collaboratively within a care team High school diploma required; related coursework or certification preferred Schedule Full-time Day shift (specific hours discussed during interview) What We Offer Competitive pay starting at $23/hour DOE Comprehensive benefits package Supportive, team-oriented work environment Opportunity to make a meaningful impact every day PACS is revolutionizing the delivery and leadership of post-acute healthcare and support services across the country.
