Social Services Director Oakwood Gardens Care Center Location: Fresno, CA Starting Pay: $30/hour Status: Full-Time Oakwood Gardens Care Center is seeking a Social Services Director to join our compassionate and dedicated team. This is a key role in helping our residents and their families navigate the challenges of long-term care, improve quality of life, and access essential resources. Key Responsibilities Establish a course of action by exploring options and setting goals with residents and families. Assist residents in achieving the highest practicable level of self-care, independence, and well-being. Provide medically related social services to support each residents physical, mental, and psychosocial health. Evaluate social, family, psychological, and emotional needs; develop and implement individualized care plans. Provide emotional support for residents and families, including coping with disabilities, health concerns, or grief. Assist residents with healthcare decisions, personal shopping, and inventory/tracking of personal belongings. Coordinate response to lost or missing belongings and facilitate community/social service resources as needed. Arrange for prosthetics, assistive devices, and ancillary services (optometry, podiatry, dental, psychiatric). Lead discharge planning with agencies and individuals to ensure smooth transitions. Maintain detailed documentation of resident social services status and progress. Collaborate with the interdisciplinary team to meet the psychosocial needs of each resident. Provide in-service training for staff as requested. Qualifications Education: Bachelors Degree in Social Work or Human Services required; MSW preferred. Experience: At least 2 years of supervised social work experience in a healthcare setting working directly with individuals. Skills: Ability to read and interpret technical procedures, policy manuals, and communicate clearly. Strong problem-solving and organizational skills. Experience with PCC (PointClickCare) preferred. Physical Demands & Work Environment Frequent standing, walking, pushing/pulling, reaching, and communicating with residents/staff. Ability to lift up to 25 lbs. occasionally.
