You will facilitate individual and group sessions, coordinate with community agencies, and advocate for patients, ensuring a centered approach to addiction treatment. Provision of patient care is based on care management, quality/resource, stewardship, patient advocacy/customer service and role modeling. Accountable for professional practice of counseling and participation in the interdisciplinary care delivery model. Principle Responsibilities Assessment/reassessment Perform initial assessment and screen for admission to addiction services. Complete comprehensive biopsychosocial assessment of patient. Reassess patient and progress in treatment. Crisis support and management. Meet requirements for 245G and third-party payers. Treatment Planning Complete treatment plan per department process. Documentation of patient, and family if appropriate, participation in development of treatment plan. Review treatment plan with patient at regular intervals. Initiate referrals to internal/external providers and agencies. Coordinate discharge and transition planning. Provision of Treatment Facilitate group and individual counseling, as identified by treatment plan. Coordinate treatment with other community agencies/providers, when appropriate. Provide patient and family education, as needed. Other duties as assigned. Required Qualifications Associates degree in Behavioral Sciences Preferred Qualifications Bachelor's degree in Behavioral Sciences Experience in a treatment program Licenses/Certifications Licensed Alcohol and Drug Counselor - MN Board of Behavioral Health & Therapy required upon hire Must meet at least ONE of the requirements below: Current BLS certification from the American Heart Association Current BLS certification from the American Red Cross Allina in-house BLS training (within 30 days of hire Physical Demands Sedentary: Lifting weight up to 10 lbs. occasionally, negligible weight frequently Pay Range Pay Range: $25.50 to $34.94 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
