Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Responsibilities: The Social Worker is responsible for daily oversight of the Housing Project which includes tracking, and outreach services in partnership with community housing providers and, if appropriate, community agencies to develop transitional housing for high risk patients who, due to their medical or behavioral health condition, have difficulty transitioning safely from a hospital into the community. ESSENTIAL FUNCTIONS In collaboration with URMC staff, facilitate enrollment of eligible patients into the shelter beds, communicate purpose of project to patients and staff and provide limited direct clinical interventions as needed to advance the project goals. Social Worker will be responsible for collecting project data, completing progress reports and collaborating with other project partners in implementing and evaluating program services. Reporting of required project metrics, utilization, productivity, effectiveness and quality. Monitor quality of program and success in engaging member in community care, establishing long term safe housing and social stability and reducing avoidable hospitalizations. Provide oversight of referrals, LOS and throughput of DePaul Housing Project (10 beds) and Catholic Charities Family and Community Services Transitional Housing beds (20 beds). Participates in community housing initiatives representing URMC. Handles risk management issues, in close consultation with his/her supervisor and the Clinical Manager Provides social work coverage, as requested, to other clinical units or programs during co-workers absence Completes the following Administrative Functions: Completes Hospital and SW Division required in-service training programs and ongoing professional development activities including those that are designed to enhance knowledge and skills in protective work Completes required Employee Health appointments within required timeframe Attends all appropriate Social Work Division meetings including, but limited to SW Division meetings, complex case discussion, transplant staff meeting, etc Supervises social work students (MSW or BSW) Orients and provides ongoing peer support to new social work staff Other duties as assigned MINIMUM EDUCATION & EXPERIENCE Master's degree from an accredited school of social work required Or equivalent combination of education and experience LICENSES AND CERTIFICATIONS LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or If duties will be performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required The University of Rochester is committed to fostering, cultivating, and preserving an inclusive and welcoming culture to advance the Universitys Mission to Learn, Discover, Heal, Create and Make the World Ever Better.
