Job Description Summary: Minimum starting salary $18.50 / hour The Community Support Specialist (CSS) is responsible for the delivery of support services to individuals impacted by a range of behavioral health issues, including substance use, trauma and mental illness. The CSS is tasked with utilizing trauma-informed and person-centered approaches to promote maximum levels of independence and autonomy for clients served. Education: Community Support Specialist will meet any of the following educational and experience requirements: A bachelors degree in a human services field. A high school diploma with any combination of four years of higher education and/or qualifying experience in behavioral health. Responsibilities: Maintain regular patient contact to ensure that individual needs are being met. Collaborate with each patient to develop individualized person-centered plans to guide treatment Provide treatment interventions which are medically necessary, ethical and in accordance with patients individualized person-centered plan. Communicate all relevant clinical information with other UBS clinical staff and other key clinical staff regarding each clients critical care needs in a timely manner. Support coworkers and teammates in providing trauma-informed, person-centered services to clients. Collaborate effectively with colleagues, service providers and treatment team members to support clients in achieving their recovery goals. Connecting patients to community resources to assist with patients needs. Provide follow-up services for those seen. CSS will coordinate with the patients regular service provider(s) or provide continued service using evidence-based practices. Those patients without a regular provider until outpatient services are established or services are no longer necessary. Community Support Specialist shall report to and perform other work-related duties as assigned by the Assistant Director of Crisis Services and shall be under the Administration of the Director of Crisis Services. The CSS shall be cross trained for other positions within the Crisis Services department, for which they are qualified to perform based on their education and/or experience. Physical Requirements: Normal office environment. Typical demands include prolonged sitting, frequent standing, bending, stooping, and occasional lifting up to 25 pounds. Requires normal range of hearing and manual dexterity sufficient to operate keyboard, telephone, photocopier, calculator, and other office equipment as needed. Requires ability to work under stressful conditions and to work irregular hours and deal with irate individuals. TB test upon hire.
