Social Services Director Location: Tice Valley Post Acute Walnut Creek, CA Position Type: Full-Time Salary: $85,000 per year + Comprehensive Benefits Role Overview Tice Valley Post Acute is seeking a compassionate, organized, and proactive Social Services Director to join our interdisciplinary team. In this vital role, you will act as a primary advocate for our residents, ensuring their physical, mental, and psychosocial well-being is maintained at the highest practicable level. The ideal candidate thrives under pressure, maintains strict confidentiality, and possesses the emotional intelligence to navigate complex family dynamics and end-of-life care with grace and objectivity. Essential Duties & Responsibilities Resident Advocacy & Care Planning: Evaluate psychological and emotional needs to develop comprehensive care plans. Assist residents in achieving independence and self-care goals. Emotional Support: Provide counseling and support for residents and families coping with physical disabilities, health conditions, grief, and loss. Clinical Coordination: Assist with healthcare decisions and coordinate on-site ancillary services including psychiatry, dentistry, podiatry, and optometry. Discharge & Community Resource Management: Lead discharge planning by coordinating with community agencies, ensuring residents have the necessary equipment and referrals for a safe transition. Case Management: Secure prosthetics, assistive devices, and community resources to meet individual resident needs. Administrative Oversight: Document resident status accurately, manage reports of missing belongings, and provide in-service training to facility staff as needed. Collaboration: Build strong working relationships with internal departments and outside agencies to ensure seamless service delivery. Qualifications & Skills Communication: Exceptional verbal communication and active listening skills. Professionalism: Proven ability to handle pressure with objectivity and maintain high levels of confidentiality. Execution: Strong planning, reporting, and organizational skills with a proactive approach to problem-solving. Relationship Building: Demonstrated ability to manage sensitive relationships with residents and their families. Benefits Competitive annual salary of $85,000.
