Your responsibilities will include: Build meaningful relationships with residents and families and provide compassionate support. Assist residents with social, emotional, and psychosocial needs. Support care planning and help identify goals that promote independence, dignity, and quality of life. Provide emotional support to residents and families dealing with illness, disability, adjustment, grief, and loss. Assist with admissions, transfers, and discharges. Participate in discharge planning and connect residents with appropriate community resources. Interview residents, guardians, and sponsors to gather required admission information. Prepare admission packets, including resident rights, privacy notices, and admission agreements. Maintain accurate resident records and identification information. Maintain accurate bed availability records. Work closely with nursing, rehabilitation, administration, and other interdisciplinary team members. Maintain confidentiality and follow facility policies and applicable privacy requirements. Previous healthcare, skilled nursing, senior living, or social services experience preferred. PointClickCare (PCC) experience is a plus, but not required. Excellent communication and interpersonal skills. Strong organizational and documentation skills. Ability to manage multiple priorities in a fast-paced environment.
