Promote resident comfort, dignity, safety, and independence. Assist with mobility, personal hygiene, meals, repositioning, and restorative care programs. Observe residents and promptly report changes in condition to the nursing team. Follow individualized care plans and support each residents unique needs. Communication & Teamwork Build positive, professional relationships with residents, families, and healthcare team members. Collaborate with nurses, therapists, and other staff to support quality resident outcomes. Maintain accurate and timely documentation. Contribute to a positive, respectful, and resident-centered team environment. Professional Standards Follow facility policies, infection prevention protocols, and safety procedures. Protect resident privacy and maintain compliance with HIPAA requirements. Demonstrate reliability, professionalism, accountability, and a strong work ethic. Support Peninsula Post-Acutes commitment to providing exceptional skilled nursing and rehabilitative care.
