Home Health Aide

Position Overview United Health Services (UHS) is seeking a compassionate and dependable Home Health Aide to join our home care team. In this role, youll provide personal care and assist with daily activities and rehabilitative exercises, helping patients live safely and comfortably in their own homes. Primary Department, Division, or Unit: UHS Home Care Primary Work Shift: Day Regular Scheduled Weekly Hours: 40 Compensation Range: $15.00 - $20.74 per hour, depending on experience ----- Job Responsibilities: Provides hands-on personal care and assistance with activities of daily living, such as bathing, dressing, grooming, and toileting, in accordance with the patients individualized plan of care Delivers care under the supervision of a Registered Nurse, following New York State Department of Health guidelines and all agency policies and procedures Encourages and supports patients in performing tasks independently to promote self-care and maintain dignity Assists with light housekeeping tasks to ensure a clean, safe, and organized home environment that supports the patients well-being Prepares and serves meals using basic nutrition principles and modifies diet plans as directed by the Registered Nurse to meet the patients health needs Provides care using infection prevention practices and reinforces infection control education given by nursing staff Aids patients with prescribed rehabilitative exercises under the guidance of a Rehabilitation Therapist to support recovery and mobility Observes the patients physical, emotional, and behavioral condition and accurately records daily care activities and any significant changes Communicates changes in the patients condition immediately to the Registered Nurse or, if unavailable, to the Home Health Aide Supervisor Participates as a valued member of the care team, offering feedback and suggestions to improve the quality and continuity of patient care Completes all documentation in a timely, thorough, and accurate manner according to agency standards and regulatory requirements Provides compassionate, respectful, and culturally sensitive care while maintaining patient privacy, dignity, and comfort in the home setting Position Qualifications: Minimum Required: High School Diploma or GED and successful completion of a Home Health Aide program within 12 months of hire, OR Current Home Health Aide Certification (no diploma/GED required if you're currently certified) ----- Why Youll Love Working at UHS At United Health Services (UHS), we believe every connectionto patients, to purpose, to each othermakes a difference.
